The Healthier You NHS Diabetes Prevention Programme was launched in 2016 and is available for people who have been identified as at risk of developing type 2 diabetes.
Offered as a digital service or a face-to-face group service, the programme gives people support on making lifestyle changes that reduce the risk of developing type 2 diabetes.
With two million people in England at a high risk of developing the condition, the programme is part of an initiative that seeks to identify those who have an increased chance of developing type 2 diabetes, providing them with the support required to negate further complications.
The programme is delivered as part of NHS England across a framework of five providers.
If you are recognised as being at risk of type 2 diabetes you will be referred to the programme.
The criteria of the programme has also been expanded to include women who have previously been diagnosed with gestational diabetes.

The digital pathway or stream provides support, assistance and guidance through the use of technologies including:
The face-to-face service offers the same personalised support as the digital pathway but in a physical group setting.
Support is provided that covers all the foundations of good diabetes management and remission, including weight management, diet advice and support for keeping physically active.
Due to the COVID-19 pandemic, the service is now delivered via telephone or over a group video conference call.
